Frequently Asked Questions

Yes. Reserve your expertise for the patients and questions that matter most. Choose who can reach you, for what areas of expertise and what type of input you provide, from quick curbsides to formal eConsults. All requests are from verified US healthcare professionals.

Yes. Many specialists formalize and bill for consults using existing CPT code 99451, typically reimbursed at approximately $30–$50 for 5 minutes of asynchronous review and written recommendations, regardless of place of service, with additional reimbursement supported through CPT 99446–99449 and commercial insurers, sometimes exceeding $125 per consult. Medicare has reimbursed these services since 2019 with historically low denial rates (<2%).

Alternatively, consults for patients you are / will see may support higher-complexity E/M visits (e.g., a 99213 → 99214 shift) through additional medical decision-making, potentially increasing $30-50 reimbursement and 0.8-1.0 wRVUs.

You can also use Simvuly to start or expand specialty consultation services provided to affiliated organizations, referral partners, customers, and other care settings through a branded experience that helps support specialist engagement, documentation, and reimbursement.

Consults also support capitated, value-based, ACO, PACE, concierge specialty care, and other care models where reducing unnecessary referrals, transfers, admissions, and delays in care creates financial value … with or without insurance billing.

No. Simvuly is free. Create an account, build your profile, and start collaborating with colleagues in minutes.

You decide. Some questions only need a quick curbside answer. Others benefit from a more formal eConsult with  focused review of patient information and documented specialist recommendations, such as for trusted referring providers, more complex cases, or situations where formal documentation may be helpful.

Requests are matched based on specialty, expertise, clinical question, region, organization, and your preferences. Create your own shared workspaces, join existing ones, or collaborate across organizations. Requests that aren’t a good fit are automatically routed to another specialist or specialty. Requests can be limited to specific clinicians, organizations, areas of expertise, geographies, or patient populations.

You control your availability. Turn off new requests and notifications from your profile and allow requests to be routed to other specialists. For more coverage, invite other specialist colleagues to handle requests when you’re unavailable.

Yes. You do not need your practice, hospital, or organization to participate. Create your free account and start consulting right away, just as you use your phone today to call, text, or answer questions on patient cases. Over time, invite trusted referring providers, colleagues, administrative staff, and practice partners to collaborate as your network grows.

Questions that would normally be handled through a curbside, eConsult, colleague-to-colleague consultation, referral guidance, treatment recommendation, diagnostic question, or second opinion. Urgent or emergency situations will continue through existing clinical workflows.

You decide. Requests can be routed to a shared specialty pool, assigned using round-robin workflows, or escalated based on urgency and availability. Organizations can route requests to internal specialists, external specialists, or both. Specialists can further tailor their profiles by specialty and areas of expertise to receive requests that best match their interests and practice.

Yes. Many specialists consult with providers for patients across state lines. As a patient is not directly involved, these consults are generally not considered telemedicine encounters and are governed differently, so geographic restrictions may not apply.

By default, requests are matched only within states where you hold an active license. You can broaden your reach in app, joining shared workspaces, or collaborating with clinicians and organizations in other states. Clinicians should follow applicable state laws, payer policies, and credentialing requirements.

No. As a patient is not directly involved, these consults are generally not considered telemedicine encounters and are governed differently. So, Specialists can provide guidance to the treating clinician without necessarily establishing a direct patient relationship or running into other telemedicine issues. Clinicians should follow applicable state laws, payer policies, and credentialing requirements.

Build a profile around your specialties, subspecialties, and areas of expertise. Simvuly helps route relevant questions to you while increasing awareness of your expertise among referring clinicians. Boost your reach over time by inviting trusted referring providers both within and outside your organization.

No. Simvuly is designed to make specialist consultation work easier. Requesters are guided to provide relevant clinical information upfront, reducing interruptions, phone tag, and follow-up questions. AI can assist with responses and optional documentation. You can always decline and allow requests to be routed to other specialists.

Many consults help clinicians manage patients locally without an in-person specialty visit. Others help determine when a referral is appropriate, which specialist is the best fit, or what information should be gathered before the visit.

You decide how involved to become, from a quick curbside answer to a formal eConsult, referral recommendation, or seeing the patient in clinic when appropriate.

Most questions can be answered asynchronously in just a few minutes, just like the consults you’re already having today. Specialists choose when and how they participate, whether by chat, call, or written recommendation. You can answer only the requests you want, when you’re available, and pause new requests at any time.

No. Consults are informational-only, not active management. The treating physician retains full responsibility for patient care and agrees not to add the consulted physician’s name in the chart unless agreed otherwise.
